Trust Forms

After a trust officer at Peak Trust Company has reviewed a trust document and advised as to Peak’s ability to serve as trustee, the following forms will be completed with all the information needed to open the trust account at Peak. Depending on the trust, planning scenario, or needs of the grantor and beneficiaries, there may be additional documents that are required during the account opening process. Additional forms will be provided as needed by the trust officer. Grantors and beneficiaries should work with their legal counsel and the trust officer at Peak to ensure all the appropriate documents are prepared at the time the trust is established or transferred to Peak Trust Company.

Trust Account Opening The Trust Account Opening Form must be completed with all the required information to open a trust account at Peak Trust Company. When submitting the completed account opening form, please include a copy of the grantor’s current government-issued ID (e.g. passport photo page or driver’s license).
Affidavit of Solvency An Affidavit of Solvency is required to be completed by grantors creating a self-settled trust in Alaska. This is only for self-settled trusts in Alaska.
Waiver of Duty To Inform The Waiver of Duty to Inform may be used when the grantor desires to delay knowledge of the trust’s existence or assets for beneficiaries. This may be appropriate in instances where the grantor fears knowledge of a trust may adversely affect beneficiaries’ lifestyle and education choices. This is only for trusts as requested by the grantor.
W-9 A W-9 form must be completed for grantor’s who are using their social security number as the tax identification number for a grantor trust. A W-9 may also be required for beneficiaries prior to receipt of distributions from the trust.

Self-Directed IRA Forms

Traditional IRA Account Application The Traditional IRA Account Application is used to open a traditional IRA (individual retirement account) at Peak Trust Company. When submitting the completed application, please include a copy of your current government-issued ID (e.g. passport photo page or driver’s license).
Roth IRA Account Application The Roth IRA Account Application is used to open a Roth IRA (individual retirement account) at Peak Trust Company.
Traditional IRA Transfer Request The Traditional IRA Transfer Request Form is used to transfer IRA assets from another custodian to an IRA account at Peak Trust Company.
Roth IRA Transfer Request The Roth IRA Transfer Request Form is used to transfer IRA assets from another custodian to an IRA account at Peak Trust Company.
Traditional IRA Contribution Eligibility he Contribution Eligibility Form will allow you, the owner, to confirm that you are able to fund your IRA according to relevant regulations.
Roth IRA Contribution Eligibility The Contribution Eligibility Form will allow you, the owner, to confirm that you are able to fund your IRA according to relevant regulations.
Withholding Notice & Election The Withholding Notice & Election allows Peak to disclose your right to have part of your IRA distributions withheld as a way to help you prepay your income taxes. This form allows you to make the appropriate election.
W-9 A W-9 form must be completed for the IRA account owner, because the IRA will report for tax purposes under the account owner’s social security number.
